When creating a registration form for a credit-bearing experience, SNHU requires the participants first name, last name, personal email address (not work), home mailing address, cell phone, birth-date, title of the PD the participant is attending and proof of payment. Grades and registration through SNHU cannot be finalized without proof of payment or any of the other information above.
The NHLI-SNHU administrator needs to know the beginning and end date of the learning experience as these are required for SNHU to determine within which semester the experience and grades will be recorded.
